Small Diameter Bomb Lesson Learned

Image
In 2006 the United States Air Force brought in a new munition the GBU-39 and a new bombrack the Bru-61. It was the first of its kind a new kind of munition that was able to be controlled from the ground and guided into targets as required. These bombs were different on many levels besides just that one thing. These bombs were not ejected like all previous munitions in the Air Forces history. These bombs are pneumatically pushed off of the bomb rack rather than the old fashion ejection process with cartridges that look like oversized shotgun shells.      In 2015 I was on a deployment and an F-15E had two hung munitions on the CFT's loaded on the side of the aircraft. When the aircraft returned to base the crew chiefs removed all fuel from the aircraft and the weapons personal on shift removed every munition from the aircraft that required carts. It was determined at that time that since the BRU-61 didn't have carts to eject the munition on the ground. They left the B...

Aviation Accidents and the Lessons learned

Accidents and Lessons Learned Air France Flight 447 was an Airbus A330 that was travelling from Rio de Janeiro, Brazil, to Paris, France, when it crashed into the Atlantic Ocean after stalling mid-air. The flight data recorders were not recovered until May 2011. They showed that the crash was ultimately the result of the crew’s failure to respond correctly after icing of the pitot tubes caused the autopilot to disconnect - with catastrophic consequences. Having departed Rio de Janeiro-GaleĆ£o International Airport on 31 May, 2009, at 19:29 local time, the aircraft last made contact at 01:35 UTC, which was three hours and six minutes after departure. At this point, it passed into a so-called “dead zone” – that is, an area where an aircraft falls outside of the communication range of either air traffic control or other aircraft. Due to the length of the flight, each of the pilots were allocated a rest break; the captain had arranged to take the second slot and left the cockpit at 02:...
